The Transition Clinic at United Regional began in 2016 to help discharged patients with continuing medical needs. These patients are often without the financial resources and direction they need to get healthier. Providers, high-risk navigators, advocates, and more not only help patients with medical essentials but address their socio-economic needs as well. This holistic approach has proven very effective in improving health, one step at a time.

and is open: Monday – Friday 8:00am-12:00pm and 1:00pm-5:00pm
