Providing Patient Advocacy and Support
Inpatient Support
If you or your family have special concerns during your hospital stay, our social workers are here to help. They offer guidance and support in areas such as:
- Discharge planning
 - Post-hospital care coordination
 - Arranging home health services or extended care facilities
 - Referrals to community agencies
 - Counseling and financial information on Medicare, Supplemental Security Income, disability, or Medicaid nursing home care
 
To reach a social worker or inpatient care coordinator, ask your nurse to contact them or call the department directly at 940-764-8800.
Supporting your Journey Home
After hospital discharge, United Regional Care Guides provide compassionate, non-clinical support to help you transition smoothly. They’ll follow up with you weekly for five weeks to:
- Monitor your recovery
 - Help manage medications
 - Schedule follow-up appointments
 - Coordinate with your insurance
 - Arrange transportation and other resources
 
Note: Care Guides are currently not available for Labor and Delivery patients.

Once you're connected with a Care Guide, they’ll reach out by phone. You can identify their call by:
