At United Regional, we’re committed to providing excellence in health care — throughout your entire care journey.

A Growing Network of Support

Whether a patient is recovering at home or transitioning to another facility, we work to ensure they have the services, equipment, and support they need for continued healing.

What began in 2012 as a meeting of six community members has grown into a robust network of more than 90 partners. This ever-expanding group includes:

  • Nursing homes
  • Home health agencies
  • Hospice providers
  • Hospitals
  • Local nonprofits
  • Community-based organizations

Together, we host educational sessions to introduce new programs, share resources, and strengthen collaboration across the region.

Shared Goals for Better Outcomes

United Regional employees and our community partners work together to: 

  • Identify high-risk patients and create smooth transitions in care
  • Increase provider awareness of patient needs and available resources
  • Reduce 30-day hospital readmissions
  • Improve patient perception of care
