Out-of-Network Information

We do not accept out-of-network Medicare Advantage plans for non-emergency services. Other out-of-network plans will require prior authorization from your insurance company. If your insurance company does not agree to coverage, you could incur higher out of pocket costs. 

What if I recently lost coverage?

If you have recently lost insurance coverage, you have 60 days to enroll in a new plan outside of a normal open enrollment period. Some possible sources of coverage are:
