We ask every patient to review the information we have for them to help ensure that the address, phone number, insurance carrier, emergency contact, and medication and condition lists have not changed since the last appointment. It is important for your physician to know any new medications you are on that were prescribed by another physician and new medical conditions you have experienced. Also, verifying your contact and other demographic information helps us make sure any communications regarding your care reaches you in a timely manner.
